Concern didn't win a high percentage of his races, but he almost always got a piece of the purse and captured a lot of fans with his dramatic come-from-behind finishes. He reached the pinnacle of his career when he ran down dual Classic winner Tabasco Cat to win the 1994 Breeders' Cup Classic (USA-G1), securing an American sire championship for his sire Broad Brush into the bargain. Concern was unable to match his success on the track with success in the breeding shed but did get one noteworthy runner in two-time American champion steeplechaser Good Night Shirt.

A plain bay horse, Concern stood 15.3 hands and was tough, durable, consistent and sound. He was a one-dimensional closer who always came from well off the pace. Trainer Dickie Small (who had also trained the horse's sire, Broad Brush) described Concern as "a plugger" and "not very talented" but "relentless." He was known as a gentlemanly stallion while at stud.

Foaled in Maryland, Concern was bred and owned by Robert D. Meyerhoff. He was trained by Richard "Dickie" Small. He entered stud in Maryland at Northview Stallion Station in 1996 and was moved to the Oklahoma Equine Reproduction Center in Washington, Oklahoma, in 2004. He was pensioned in 2011 but continued to receive visits from fans until his death on March 26, 2015.

Fun facts
- Concern was the last of the 91 Breeders' Cup starters of 1994 to arrive at Churchill Downs, getting in at 3 a.m. the day before the race after an 11-hour van ride from his Maryland base. He was also the first Breeders' Cup horse to leave, pulling out for home just four hours after winning the Breeders' Cup Classic.
- The first Maryland-bred to win the Breeders' Cup Classic, Concern was also the first Maryland-bred to earn over US$2 million in a single year and the second Maryland-bred to amass over US$3 million in lifetime earnings.
- Concern was the third of five Breeders' Cup Classic winners for Hall of Fame jockey Jerry Bailey. The others were Black Tie Affair (1991), Arcangues (1993), Cigar (1995) and Saint Liam (2005).
- Concern was the only Breeders' Cup Classic winner to race or stand in the state of Oklahoma.
